I understand that this is a caregiver – child group program and that caregivers are responsible for supporting their child's participation, behavior, and safety during the duration of the class. I understand that the program requires participation in a small group setting*

I acknowledge that payment is required to reserve my child’s spot in the Founding Cohort.*

I understand that New Moon playgroups are enrichment-based and are not designed as behavioral management or individualized therapy services. New Moon Speech and Language reserves the right to decline or discontinue enrollment when a participant's needs are not a suitable fit for the group environment.*

I understand and agree to the participation requirement and community guidelines on the Policies page*

I consent to photos/videos being used for marketing purposes (optional)
